MARYBELL RAMIREZ, APRN-FPA, FNP-C Family Nurse Practitioner Marybell has been dedicated to patient-centered nursing care for over 15 years. She graduated from the nursing program at the City Colleges of Chicago in 2011. Earned a Bachelor of Science in Nursing from Purdue University in 2015 and became a Family Nurse Practitioner in 2018, earning a Master of Science in Nursing from Chamberlain University College of Nursing. Her clinical background includes family practice, home health care, ophthalmology, wound care, foot care, and wellness medicine, with a strong emphasis on preventive health and long-term wellness. Her approach focuses on comprehensive wellness evaluations that go beyond routine care, prioritizing early detection, preventive screenings, and proactive health management. Marybell is committed to patient education, attentive listening, and empowering individuals to make informed decisions about their health. MODERN CARE WITH A PERSONAL TOUCH Altar Healthcare is a primary care practice focused on non-emergent medical care delivered with professionalism, discretion, and individualized attention. Each patient begins with a comprehensive consultation designed to create a personalized care plan aligned with their goals, lifestyle, and health needs. Altar Healthcare represents a space where patients are supported, prioritized, and cared for with intention. It blends compassionate primary care, preventive medicine, and holistic wellness into a grounded, patient-focused approach. We accept Medicare and most major PPO insurance plans. Some services may be covered depending on insurance coverage and medical necessity. Self-pay options are also available. Patients are encouraged to obtain specific benefit information from their insurance carrier. On-site lab testing is available for added convenience, allowing patients to complete diagnostics in one location. Care options include same-day visits, telehealth appointments, and in-office services. At-home care and concierge after-hours services are available by request based on provider availability and clinical need.
CANCELLATION POLICY
We value your time and strive to provide timely care to all patients. To help us maintain availability, we kindly ask that you provide at least 24 hours’ notice if you need to cancel or reschedule your appointment. Appointments canceled with less than 24 hours’ notice, as well as missed appointments (“no-shows”), may be subject to a cancellation fee. Patients arriving more than 10–15 minutes late may need to reschedule, depending on provider availability. We understand that unexpected situations can arise.
